About this calculator
The urine albumin-to-creatinine ratio (UACR, also written ACR) detects small amounts of albumin leaking through damaged kidney filters. It is the preferred screening test for kidney damage in people with diabetes or hypertension and is a key part of chronic kidney disease staging.
Dividing albumin by creatinine corrects for how dilute the urine is, so a single spot sample can be interpreted without a 24-hour collection.
How it works
UACR (mg/g) = urine albumin (mg/L) ÷ urine creatinine (g/L)
UACR (mg/mmol) = UACR (mg/g) ÷ 8.84
Urine creatinine in mg/dL is multiplied by 0.01 to get g/L. 1 mmol/L creatinine equals 11.312 mg/dL.
How to read your result
| Category | mg/g | mg/mmol | Meaning |
|---|---|---|---|
| A1 | < 30 | < 3 | Normal to mildly increased |
| A2 | 30–300 | 3–30 | Moderately increased |
| A3 | > 300 | > 30 | Severely increased |
Combine with eGFR to place a person on the KDIGO risk heat-map.
Worked example
Urine albumin 45 mg/L and urine creatinine 110 mg/dL.
| UACR | 40.9 mg/g Also 4.6 mg/mmol |
| Albuminuria category (KDIGO) | A2 Moderately increased (formerly microalbuminuria) |
Frequently asked questions
What is a normal UACR?
Below 30 mg/g (3 mg/mmol) is normal to mildly increased.
What causes a falsely high UACR?
Exercise, fever, urinary infection, menstruation, heart failure and marked hyperglycaemia can transiently raise albumin excretion.
Which sample is best?
A first-morning spot urine is preferred. Random samples are acceptable if first-morning is not available.
What is the difference between UACR and UPCR?
UACR measures only albumin. UPCR measures total protein and is used when non-albumin proteins are suspected.
Limitations and when not to use
Screening aid only. Interpretation requires clinical context and repeat testing.
